The Nevada Physician and Chiropractor Progress Report Certification of Disability is a form used to assess the progress of an individual who has been disabled due to an injury or illness in the state of Nevada. It is used by physicians and chiropractors to provide an assessment of the disability and determine the level of care and treatment needed. This form is required for disability benefits from the state of Nevada and is typically part of an application packet. There are two types of Nevada Physician and Chiropractor Progress Report Certification of Disability: the first is a Functional Capacity Evaluation (FCE) which assesses an individual’s ability to perform basic activities of daily living and work-related tasks; and the second is an Impairment Rating (IR) which is used to assign a numerical value to a disability based on a medical diagnosis. Both of these assessments are used to determine the level of disability and the amount of benefits an individual is eligible to receive.
