Additionally, the notice must be signed by the policyholder and the insurer. The Cancellation Notice is used to cancel an insurance policy before it expires. It must include the policyholder’s name, address, policy number, and the date of cancellation. The notice must also be signed by both the policyholder and the insurer. The Reinstatement Notice is used to reinstate an insurance policy after it has been terminated or canceled. It must include the policyholder’s name, address, policy number, and the date of reinstatement. The notice must also be signed by both the policyholder and the insurer.
