The Nevada OD-4 Limited Heart Examination Form is a state-mandated form used by Nevada-licensed physicians to perform required medical examinations for individuals applying for motor vehicle operator's licenses. The form includes basic information about the applicant and an assessment of their cardiovascular health. The Nevada OD-4 form is divided into two parts: Part A is used to assess the applicant's medical history and Part B is used to evaluate any symptoms or abnormalities that may indicate cardiovascular disease. The form includes questions about the applicant's medical history, family history of heart disease, and any medications they are currently taking. It also contains sections for physical examination, electrocardiogram (ECG), and echocardiogram (echo). The Nevada OD-4 form must be completed and signed by the examining physician prior to the issuance of a driver's license.
