New York Verification (Form 4-11) is a form filled out by employers to verify the identity and work authorization of an employee in the state of New York. The form is used to comply with New York State’s Labor Law, Article 2, Section 225-a, which requires employers to display a poster informing employees of their right to work in the United States. New York Verification (Form 4-11) requires employers to provide information on the employee’s identity, such as name, date of birth, place of birth, and Social Security number, as well as their work authorization, such as their visa or green card number. There are two types of New York Verification (Form 4-11): a version for employers and a version for employees. The employer version is used to document the employee’s identity and work authorization, while the employee version is used to verify the accuracy of the information provided by the employer.
