New York RADIO Form A p3 is an official form utilized by the New York State Department of Taxation and Finance to apply for a Certificate of Authority to do business in the State of New York. It is also known as a Business Corporation Franchise Tax Return (Form CT-3). There are three types of New York RADIO Form A p3: Form CT-3, Form CT-3-ATT, and Form CT-3-ATT-I. Form CT-3 is the main form which requires the filer to provide information about their business such as its legal name, address, type of business, and estimated annual gross receipts. Form CT-3-ATT is an attachment to Form CT-3, and requires the filer to provide additional information such as the name and address of any parent corporation, affiliated corporations, and/or subsidiaries. Form CT-3-ATT-I is an additional attachment to Form CT-3-ATT and requires the filer to provide additional information such as the number of shares of each class of stock issued and outstanding and the names and addresses of all shareholders who own ten percent or more of the stock.
