New York Initial Report of Guardian is an annual report published by the New York State Office of Children and Family Services (OFFS). The report is mandated by New York State Social Services Law and is intended to help guardians of minors in New York State understand their legal obligations and duties as a guardian. The report includes information regarding the guardianship process, factors to consider when making decisions about a child's welfare, the rights and responsibilities of guardians, the court process for guardianship, and other related topics. There are two types of New York Initial Report of Guardian: (1) the General Report, and (2) the Special Report. The General Report is intended for guardians of minors in general, and the Special Report is intended for guardians of minors with special needs or circumstances. Both reports provide information and guidance for guardians to help them make informed decisions and fulfill their legal duties and responsibilities.
