A New York Notice of Appeal and Affidavit of Service of Notice SORTRA Order, is a legal document filed with the court when a party wishes to appeal the decision of a lower court or administrative agency. This document must be served upon all parties involved in the case. There are two types of New York Notice of Appeal and Affidavit of Service of Notice SORTRA Order: 1) Civil Notice of Appeal and Affidavit of Service of Notice SORTRA Order, and 2) Criminal Notice of Appeal and Affidavit of Service of Notice SORTRA Order. The Civil Notice of Appeal and Affidavit of Service of Notice SORTRA Order is used when appealing a civil decision, such as a family court order or a decision by a state or local government agency. The Criminal Notice of Appeal and Affidavit of Service of Notice SORTRA Order is used when appealing a criminal conviction or sentence. In both types of notices, the filing party must provide information on the court or agency from which the appeal is being taken, the date of the decision being appealed, the names of the parties involved, and a brief description of the nature of the appeal. The Notice must also be served upon all parties involved in the case. The Affidavit of Service of Notice SORTRA Order must be completed and included with the Notice of Appeal. This affidavit serves as proof that the Notice was properly served upon all parties.
