A New York Affidavit (Change of circumstances since certification as qualified adoptive parent) is a legal document used by those who have been certified as qualified adoptive parents in New York to report any changes in their circumstances since the certification. This affidavit must be completed and filed with the New York State Office of Children and Family Services (OFFS). There are three types of New York Affidavit (Change of circumstances since certification as qualified adoptive parent): 1) Change of Employment/Income, 2) Change of Residence, and 3) Transfer of Case. The Change of Employment/Income affidavit must be completed when there is a change in the adoptive family’s employment or income. This includes changes in job title, salary, or hours worked. The Change of Residence affidavit must be completed when there is a change in the adoptive family’s residence, such as moving to a different address. The Transfer of Case affidavit must be completed when an adoptive family wishes to transfer their adoption case to another agency or attorney. All New York Affidavit (Change of circumstances since certification as qualified adoptive parent) must be signed by all adult members of the adoptive family and witnessed by two adults. The affidavit must then be mailed or hand-delivered to the OFFS.
