The New York Broker-Dealer and Securities Registration Information Sheet (also known as the Form BD or Form BDW) is a filing document used by broker-dealers to register with the New York State Department of Financial Services (DFS). The Form BD provides key information about the broker-dealer, including its business activities, financial condition, and the qualifications of its officers, directors, and associated persons. The Form BD also serves as the broker-dealer’s initial registration document with the DFS. There are two types of New York Broker-Dealer and Securities Registration Information Sheet: a Standard Form BD and a Form BDW. The Standard Form BD is used by broker-dealers who are registered with the Securities and Exchange Commission (SEC) or a self-regulatory organization (SRO). The Form BDW is used by broker-dealers who are not registered with the SEC or an SRO, and instead are registered with the DFS. Both forms require the broker-dealer to provide information about its business activities, financial condition, and the qualifications of its officers, directors, and associated persons. The Form BDW also requires the broker-dealer to disclose information about its ownership, investments, and other material facts. Both forms must be updated annually, and the broker-dealer must also inform the DFS of any material changes to the information in the form.
