The New York Payment Advices Cover Sheet is a document used to accompany all payments and invoices that are to be sent to the New York State Comptroller's Office. The form is used to provide the Comptroller with information about the payment or invoice and contains the payee's name, address, Taxpayer Identification Number (TIN), and the purpose of the payment. There are two types of New York Payment Advices Cover Sheet: the Payment Advices Cover Sheet for Payments and the Payment Advices Cover Sheet for Invoices. The Payment Advices Cover Sheet for Payments is used for all types of payments, such as payroll, vendor payments, and other payments. The Payment Advices Cover Sheet for Invoices is used for all invoices, such as those from vendors, contractors, and other businesses.
