The New York Addendum to Family Protection Registry Information Sheet is a document that provides additional information to the Family Protection Registry Information Sheet (FP RIS). The addendum is a form required to be filed with the FP RIS in order to legally serve process in certain counties in New York State. There are three types of New York Addendum to Family Protection Registry Information Sheet: General Addendum, Nassau County Addendum, and Suffolk County Addendum. The General Addendum is used for all other counties in New York State and requires the filing of a separate addendum for each county in which service of process is to be made. The Nassau County Addendum is used when service of process is to be made in Nassau County and requires the filing of a separate addendum for each party to be served. The Suffolk County Addendum is used when service of process is to be made in Suffolk County and requires the filing of a separate addendum for each party to be served. The New York Addendum to Family Protection Registry Information Sheet includes information such as the name of the person to be served, the address of the person to be served, the name of the court, and the date of service.
