The New York Affidavit of Military Investigation (CIV-LT-106) is a legal document used to verify the military service of a person who is applying for various benefits or privileges in the state of New York. It is a sworn statement that must be signed by a military official confirming the applicant's service. The Affidavit of Military Investigation is sometimes referred to as an “Affidavit of Service” or “Affidavit of Military Status” and is typically required for benefits such as veterans' benefits, veterans' housing, veterans' employment, veterans' educational assistance, and veterans' health care. There are two main types of New York Affidavit of Military Investigation: an Active Duty Affidavit (CIV-LT-106A) and a Reserve Component Affidavit (CIV-LT-106R). The Active Duty Affidavit is used for applicants who have served or are currently serving in the active duty component of the United States Armed Forces, while the Reserve Component Affidavit is used for applicants who have served or are currently serving in the Reserve or National Guard components of the United States Armed Forces.
