A New York Affidavit in Support of an Order for Withdrawal of Funds is a document that is used to provide evidence to a court to allow a person to withdraw funds from a financial institution. This affidavit can be used in cases involving guardianship, conservatorships, or other situations where a court has issued a written order allowing the withdrawal of funds. The affidavit must be sworn to by the person who is requesting the funds and must include the following information: the court order authorizing the withdrawal, the name and address of the financial institution, the account number of the account from which the funds are to be withdrawn, the amount of the withdrawal, and the purpose of the withdrawal. There are two types of New York Affidavit in Support of an Order for Withdrawal of Funds: a General Affidavit and a Limited Affidavit. The General Affidavit is used when a financial institution is holding funds in excess of the amount specified in the court order and the Limited Affidavit is used when a financial institution is holding funds that are equal to or less than the amount specified in the court order.
