The New York Eviction Intake Sheet is a legal document used by tenants and landlords in the state of New York to initiate the eviction process. It outlines the tenant's name, address, and contact information; the landlord's name and address; the reason for the eviction; the amount of rent due; and any other relevant information. There are two types of New York Eviction Intake Sheet: The Summary Nonpayment of Rent Eviction Proceeding and the Holdover Eviction Proceeding. The Summary Nonpayment of Rent Eviction Proceeding is used when a tenant has failed to pay rent and the landlord seeks to evict the tenant. The Holdover Eviction Proceeding is used when a tenant has violated the terms of their lease and the landlord seeks to evict the tenant. Both forms require the landlord to provide the tenant with ten days of written notice before filing the eviction papers with the court.
