The New York Certificate of Termination of Existence (Of Foreign Corporation) Cover Sheet is a document that must be submitted to the New York Department of State (NYS DOS) when filing a Certificate of Termination of Existence (also known as an Articles of Dissolution, Certificate of Dissolution, or Certificate of Cancellation) for a foreign corporation. The cover sheet must be filed in addition to the Certificate of Termination of Existence form and any required documents.
