There are two types of New York Articles of Dissolution (Domestic LLC), namely short form and long form. The short form Articles of Dissolution must include the name of the LLC, date of dissolution, and signature of a representative of the LLC, and must be filed with the Department of State. The long form Articles of Dissolution must include the name of the LLC, date of dissolution, signatures of all members of the LLC, and the reason for dissolution, and must be filed with the Department of State. Both forms of Articles of Dissolution must be notarized.
