The New York Certificate of Amendment of Certificate of Assumed Name Form is a legal document that is used to register a business name change in the state of New York. Filing an amendment to an assumed name allows businesses to update their previously registered assumed name with the New York Secretary of State. There are three types of New York Certificate of Amendment of Certificate of Assumed Name Form: Form DOS-1345-a, Form DOS-1345-b, and Form DOS-1345-c. Form DOS-1345-a is used to amend the original application for the assumed name; Form DOS-1345-b is used to add a location to the assumed name; and Form DOS-1345-c is used to delete a location from the assumed name. The form must be completed in its entirety and signed by an authorized individual, and then it must be filed with the New York Secretary of State’s office. Once the form is approved, the updated information will be reflected in the state records.
