The New York Impaired Driver Program Application is an online form mandated by the New York State Department of Motor Vehicles. This form is required for individuals seeking to enroll in the Impaired Driver Program, and is used to determine eligibility for the program. The program is designed for individuals who have been convicted of a driving offense related to impaired driving, such as Driving While Intoxicated (DWI), Driving While Ability Impaired (DWI), or Driving Under the Influence (DUI). The application includes detailed personal information, including name, address, driver’s license number, date of birth, and other pertinent details. It also includes information about the individual’s driving history, including any previous impaired driving offenses, suspensions, and revocations. The application asks applicants to provide a detailed description of the incident that led to their conviction and the circumstances leading up to it. Two types of New York Impaired Driver Program Applications exist: the Standard Impaired Driver Program Application and the Enhanced Impaired Driver Program Application. The Standard Impaired Driver Program Application is used for individuals who are applying for the Impaired Driver Program for the first time. The Enhanced Impaired Driver Program Application is used for individuals who have completed the Impaired Driver Program in the past but have been convicted of another impaired driving offense within the last five years.
