New York Supplement to: Medical Examination of Driver Report Under Article 19-A is a form used by the New York State Department of Motor Vehicles (DMV) to record medical information related to a driver's fitness for duty. The form is used to evaluate a driver's physical and mental health and certify the driver’s ability to safely operate a motor vehicle. The form is divided into three parts: A, B, and C. Part A is the Driver's Medical History, which includes a Physical Exam, Vision Exam, and Mental/Neurological Exam. Part B is the Medical Examiner's Certification, which includes the Medical Examiner's signature and date of the examination. Part C is the Medical Examiner's Report, which includes the driver's medical condition, any special requirements for the driver to be medically qualified, and any other necessary medical information. There are two types of New York Supplement to: Medical Examination of Driver Report Under Article 19-A: a Standard form and a Diabetes form. The Standard form is used for all drivers and the Diabetes form is used for drivers with diabetes.
