New York Affirmation of Service (Notice of Motion and Affidavit or Affirmation in Support) is a legal document used in the state of New York to initiate a court proceeding. This document is typically filed with the court clerk, along with a summons, to begin the litigation process. It is also called a “Notice of Motion and Affidavit or Affirmation in Support”. The New York Affirmation of Service (Notice of Motion and Affidavit or Affirmation in Support) includes the names of the parties involved in the dispute, the court in which the action is to be filed, and the requested relief sought. It also includes the statement of the grounds upon which the action is based, the specific facts alleged, and the relief sought. There are three types of New York Affirmation of Service (Notice of Motion and Affidavit or Affirmation in Support): 1) Notice of Motion and Affidavit in Support of a Motion; 2) Notice of Motion and Affirmation in Support of a Motion; and 3) Notice of Motion and Affidavit in Support of a Motion for Summary Judgment.
