New York Notification of Confidentiality Issue or Error is a notification issued by the New York State Department of Health when a health care provider or insurer discovers that a patient’s confidential information has been disclosed or shared inappropriately. This notification informs the patient of the issue and provides the patient with information on how to protect their rights and seek redress. There are three types of New York Notification of Confidentiality Issue or Error: Unauthorized Disclosure, Unauthorized Access, and Disclosure Error. Unauthorized Disclosure occurs when confidential information is released to an unauthorized third party. Unauthorized Access occurs when someone without authorization gains access to private information. Disclosure Error occurs when confidential information is inadvertently shared with an unauthorized third party.
