The New York Report of Unauthorized Use of License/Registration (Instructions Included) is a form used to report cases of misuse of a driver's license or vehicle registration. It is intended for use by law enforcement, insurance companies, or other parties that have a legitimate need to know about such misuse. The form contains instructions on how to properly fill out the report, including what information must be provided, such as the name and address of the person whose license or registration has been misused. It also includes space for an explanation of the alleged misuse, as well as an affidavit that can be signed by the person filing the report. There are three types of New York Report of Unauthorized Use of License/Registration (Instructions Included): Form MV-567, Form MV-567A, and Form MV-567B. Form MV-567 is used for reporting the misuse of a driver's license, while Form MV-567A and Form MV-567B are both used for reporting the misuse of vehicle registration.
