The New York Affidavit In Support of Entry of Out-of-State Order of Protection or Temporary Order of Protection Onto Statewide Registry of Orders of Protection is a form that is used by individuals who have an out-of-state order of protection or temporary order of protection and wish to have it entered onto the New York Statewide Registry of Orders of Protection. This is an important document that is used for the enforcement of a valid out-of-state order of protection. There are two types of New York Affidavit In Support of Entry of Out-of-State Order of Protection or Temporary Order of Protection Onto Statewide Registry of Orders of Protection available: the CPL-T-21 form and the CPL-T-21.1 form. The CPL-T-21 form is used for entry of an out-of-state order of protection onto the New York Statewide Registry, while the CPL-T-21.1 form is used for entry of a temporary order of protection onto the New York Statewide Registry. Both forms require the individual filing the affidavit to provide information about the out-of-state order of protection or temporary order of protection, and the petitioner and respondent involved in the case. In addition, the affidavit must be signed under penalty of perjury.
