The New York Request for Premises Check (RPC) is a form designed by the New York State Office of Mental Health to help clinicians assess the safety of a person's home or living environment. PCs are used to ensure that a person's home is free from hazards and risks that may be related to mental health issues. The RPC includes questions about the living situation, such as whether the person is living alone, whether there is a locked door or windows, and whether the person has access to appliances and other safety measures. It also includes questions about violence, drug use, and access to other resources. The RPC is intended to be a comprehensive tool for evaluating the safety of a home and can be used to identify potential mental health risks. There are two types of New York Request for Premises Check: the Short Form and the Long Form. The Short Form is used when a clinician needs to quickly assess a person's living environment. The Long Form is used when a more comprehensive assessment is needed.
