The New York Statement of Identity by Parent/Guardian is a document issued by the New York State Department of Health (DOH) that verifies the identity of a child or minor under the age of 18. This document is required to obtain a birth certificate, a driver’s license, or a passport. The Statement of Identity is also used in cases of adoption, guardianship, or when a minor needs to prove their identity. The Statement of Identity is signed by the child’s parent or guardian and is accompanied by additional documents such as a birth certificate, proof of residence, and a photo ID. There are two types of New York Statement of Identity by Parent/Guardian: 1. The Statement of Identity for Children: This document is for children under the age of 18 and requires the parent or guardian’s signature as well as additional documents such as a birth certificate, proof of residence, and a photo ID. 2. The Statement of Identity for Minors: This document is for minors between the ages of 18 and 21 and requires the parent or guardian’s signature and additional documents such as a birth certificate, proof of residence, and a photo ID.
