The New York Statement of Identity — For Applicants Represented by Government or Government-Approved Facilities is a legal document that must be completed by all applicants seeking a state identification card or driver’s license in New York. This form requires the applicant to provide information about themselves to verify their identity, such as their name, date of birth, Social Security number, and address. In addition, applicants must also provide a signature, photograph, and other proof of identity, such as a birth certificate or passport. The form must be submitted to the New York Department of Motor Vehicles (DMV) or the local government office in order to be processed.
