The New York Application for Title is an official form issued by the New York State Department of Motor Vehicles (DMV). The form requires information about the vehicle, including the Vehicle Identification Number (VIN), year, make, model, body style, and odometer reading. The buyer and seller must provide their contact information, as well as any lien holder information, if applicable. The form must also be signed and notarized by both the buyer and seller.
