The New York Social Security Appeal Consent/Declination Form is a document used in the state of New York to appeal decisions made by the Social Security Administration. This form allows individuals to accept or decline an appeal request made by the Social Security Administration. There are two types of New York Social Security Appeal Consent/Declination Forms: one for the Appellant (the individual appealing the decision) and one for the Appealed (the individual or party defending the decision). The Appellant form requires the Appellant to provide personal information, such as name, address, and Social Security Number, as well as the reason for the appeal and the requested action. The Appealed form requires the Appealed to provide information about the decision that is being appealed, as well as the reasons for the appeal request. Both forms must be signed and dated by the Appellant and Appealed in order to be valid.
