New York Application for Initial Registration or Transfer of Ownership of Pharmacy is an application form that must be completed and submitted to the New York State Department of Health in order to register or transfer ownership of a pharmacy in the state of New York. There are two different types of application forms: one for initial registration and one for transfer of ownership. The Initial Registration application requires the applicant to provide information about the pharmacy, including its name, location, type of business, and contact information for the owner and/or pharmacist. The applicant must also include a copy of their license or permit to operate a pharmacy in the state, proof of insurance, and a list of drugs and supplies to be sold. The Transfer of Ownership application requires the applicant to provide information about the current owner and/or pharmacist, the new owner and/or pharmacist, and any changes to the pharmacy's name, address, or type of business. The applicant must also include a copy of the current owner's license or permit to operate a pharmacy in the state, proof of insurance, and a list of drugs and supplies to be sold. Both applications must be accompanied by the appropriate fee and must be signed and dated by the applicant or their authorized representative. Upon submission, the application will be reviewed by the New York State Department of Health and, if approved, the pharmacy will be registered or transferred.
