The New York UCS-111 Child Support Summary Form is a document used by the New York State Division of Child Support Enforcement to summarize and calculate child support payments. The form is used to determine the total amount of child support owed and to help ensure that the payments are made in a timely manner. The form is divided into three parts: the Summary of Support Obligation, the Summary of Payments Received, and the Summary of Arrears Owed. The Summary of Support Obligation portion of the form includes the total amount of child support due each month, the amount of arrears due, and the amount of arrears owed to the custodial parent. The Summary of Payments Received section lists the total amount of payments received by the custodial parent, as well as the date of receipt. Finally, the Summary of Arrears Owed section outlines the total amount of arrears owed to the custodial parent. There are two types of New York UCS-111 Child Support Summary Form: the Standard form and the Modified form. The Standard form is used when the child support agreement is based on the New York State Child Support Standards Act. The Modified form is used when the child support agreement is based on the New York State Family Court Act.
