The New York Affidavit of Personal Service is a sworn statement used to prove that a document was delivered in person to the other party in a legal case. It is also known as an “Affidavit of Service by Personal Delivery” or an “Affidavit of Hand Delivery.” This document can be used in any court in New York State and is typically filed with the court after the document has been served. There are two types of New York Affidavit of Personal Service. The first type is an “Affidavit of Service of Process” which is used when a process server is hired to serve the document. The process server must fill out the affidavit and attach a copy of the document they served to it. The second type is an “Affidavit of Service by Hand Delivery” which is used when the document is delivered directly to the other party. In this case, the person delivering the document must fill out the affidavit and attach a copy of the document they served. In both cases, the person filling out the affidavit must list the name and address of the other party, the date and time the document was delivered, and the manner in which it was delivered. The affidavit must be signed in front of a notary public and filed with the court.
