New York Application For Index Number is a form issued by the Department of Taxation and Finance of the State of New York. It is used to obtain an identification number for individuals and businesses that need to file taxes in the state. The form is also known as the Form IT-2104. There are two types of New York Application For Index Number. The first is the Form IT-2104-E, which is used by individuals who are filing taxes as a sole proprietor, partner, or LLC member. The second is the Form IT-2104-C, which is used by businesses that are filing taxes as a corporation or LLC. The form requires information such as the name, address, and Social Security Number of the applicant, as well as the business’s Federal Employer Identification Number (VEIN). It also requires a signed statement from the applicant certifying the accuracy of the information provided. Once the form is completed and submitted, the New York Department of Taxation and Finance will issue an Index Number to the taxpayer. This number is used to identify the taxpayer when filing taxes in the state.
