The second is the Unilateral Dissolution of Domestic Partnership Affidavit which is completed and signed by one partner in the domestic partnership. Depending on the circumstances, one or both forms may be required to dissolve the domestic partnership. Both forms must include the full legal names of the partners, the date when the domestic partnership was established, and the current address of each partner. Once the form is completed and signed, either partner may file the form with the New York State Department of Health. The filing fee may vary depending on the county in which the form is filed.
