New York Commercial Small Claims Complaint Form Instructions are a set of instructions to help filers submit a small claims action in New York State. The form must be completed by the plaintiff (the person filing the complaint) and is used to initiate a legal action in small claims court. It contains various sections for the plaintiff to provide the court with information about the dispute, as well as a section for the defendant (the person being sued) to provide their contact information. There are two types of New York Commercial Small Claims Complaint Forms: one for businesses and another for individuals. The business form is used for claims of up to $5,000, while the individual form is used for claims up to $3,000. Both forms require the plaintiff to provide the court with identifying information, such as the plaintiff’s name, address, and phone number; a description of the dispute; and a statement of damages sought.
