The New York Request for Certified Transcript of Record or Certificate of Disposition is a document which is used to request an official certified copy of a criminal history record or the disposition of a criminal court case from the New York Department of Criminal Justice Services (CJS). This document can be used by individuals or organizations to obtain official copies of court records and dispositions for criminal cases in the state of New York. There are two types of New York Request for Certified Transcript of Record or Certificate of Disposition: 1) Request for Certified Transcript of Record which is used to obtain an official copy of the court record of a criminal case; and 2) Request for Certificate of Disposition which is used to obtain an official copy of the disposition of a criminal court case. The document requires the completion of information such as the name and date of birth of the person being requested, the case number, the court location, and a signature of the person making the request. Once the request is submitted, the CJS will process the request and send the certified transcript or certificate of disposition to the requester.
