New York Annex A (Additional Sheet — Preliminary Conference Order-Stipulation) is a form used in New York State Supreme Court proceedings. It is used by lawyers for their clients in order to provide the court with a plan for the parties to cooperate in the resolution of a legal dispute. The form is typically used in cases involving family law, divorce, and child custody. There are two types of New York Annex A: Additional Sheet — Preliminary Conference Order-Stipulation and Additional Sheet — Stipulation of Settlement. The Additional Sheet — Preliminary Conference Order-Stipulation is used prior to the court entering a Preliminary Conference Order, and is used to outline the agreement between the parties for the purpose of resolving the dispute. The Additional Sheet — Stipulation of Settlement is used to outline the agreement between the parties after a Preliminary Conference Order has been entered. Both forms must be signed by all parties to the agreement and filed with the court.
