The Ohio Application For Expungement Of Record Involving Conviction is an official form that must be filed with the Ohio court system to request the expungement of a criminal conviction from an individual's record. This form is available online and must be completed in full in order for the expungement process to begin. Depending on the type of conviction, there are two types of Ohio Applications for Expungement of Record Involving Conviction: (1) Application for Expungement of Felony/Misdemeanor Record; and (2) Application for Expungement of Felony/Misdemeanor Conviction Record. The first type is used for requesting the expungement of any conviction that was not a felony or misdemeanor. The second type is used for requesting the expungement of felony and/or misdemeanor convictions. Both types of applications require the individual to submit the completed application, along with any other required documents, to the appropriate court in order for the expungement process to begin.
