The Ohio Sharps Injury Form Needle stick Report (also known as the Ohio Exposure Incident Report) is a form used to report any needle stick, laceration, or other puncture wound where a sharp object, such as a needle or scalpel, has penetrated the skin of an employee in Ohio. This form is required by Ohio law and must be filled out and sent to the Bureau of Workers’ Compensation following an incident. The report should include details about the incident, including the date, type of injury, location, and other pertinent information. There are two types of Ohio Sharps Injury Form Needle stick Report: the Incident Report and the Follow-Up Report. The Incident Report is completed at the time of the incident and must be sent to the Bureau of Workers’ Compensation within 15 days. The Follow-Up Report is completed after the employee has completed medical treatment and must be sent to the Bureau within 30 days of the injury.
