It is available from the Ohio Department of Job and Family Services. There are three types of Ohio Application For Death Benefits And Or Funeral Expenses: Ohio Death Certificate Application, Ohio Funeral Expense Assistance Program Application, and Ohio Funeral Expense Pre-seed Contract. The Ohio Death Certificate Application allows the family to request a certified copy of the death certificate. The Ohio Funeral Expense Assistance Program Application is used to apply for financial assistance for funeral expenses. The Ohio Funeral Expense Pre-seed Contract is required if the family wishes to enter into a pre-seed funeral contract.
