Ohio Parental Waiver, Release of Liability, Indemnification and Consent for Baseball Roster is a legal document that releases a youth baseball team from liability in the event of an injury or accident to its players. It is also a way for parents to give their consent for their child to participate in the sport. There are three different types of waivers for this purpose: Waiver of Liability, Release of Liability, Indemnification and Consent Form. The Waiver of Liability releases the team and its coaches from any legal action that may arise from participation in the sport. The Release of Liability releases the team from any responsibility for any injuries that may occur as a result of the sport. The Indemnification and Consent Form releases the team from any financial responsibility for such injuries. All three forms must be signed by the parent or legal guardian of the minor child in order for the child to be eligible to participate in the team.
