The form is used in cases where the court finds that the child’s health or safety is in danger due to the parent’s lack of willingness or ability to provide a safe and nurturing environment. The form is used to permanently terminate the parent’s rights and responsibilities towards the child, including custody, visitation, and financial support. There are two types of Oklahoma Verdict Forms- Termination of Parental Rights for Failure to Correct Conditions: one for non-Indian children, and one for Indian children. The form for non-Indian children includes the court’s findings of fact, conclusions of law, and order. The form for Indian children includes additional information such as the tribe’s findings of fact and conclusions of law, and any special considerations for the Indian child.
