This form must be completed and signed by both the employer and the employee. There are two types of Oklahoma SCOPE OF AUTHORITY OR EMPLOYMENT --- DEPARTURE: voluntary and involuntary. Voluntary Departure occurs when an employee resigns from their job of their own free will. In this situation, the employer and employee must both sign the Oklahoma SCOPE OF AUTHORITY OR EMPLOYMENT --- DEPARTURE form to formally document the employee's departure. Involuntary Departure occurs when an employee is terminated from their job due to misconduct or a violation of company policy. In this situation, the employer must sign the Oklahoma SCOPE OF AUTHORITY OR EMPLOYMENT --- DEPARTURE form and the employee must receive a copy of the form. The form must also include the reasons for the termination.
