Oklahoma Verification of Matrix is an annual process used by the Oklahoma Department of Public Safety to verify individuals’ identity and eligibility to obtain an Oklahoma driver’s license or identification card. The process involves verifying the individual’s name, address, date of birth and Social Security number against the federal Social Security Administration’s database. Oklahoma Verification of Matrix is composed of three types of verification: identification, address and Social Security number. Identification verification requires an applicant to provide two valid forms of identification, such as a driver’s license, state-issued identification card, passport or birth certificate. Address verification requires an applicant to provide proof of address with two documents, such as a utility bill, lease agreement or bank statement. The Social Security number verification requires an applicant to provide their Social Security number and answer questions about their employment history. Upon completion of the Oklahoma Verification of Matrix process, applicants will receive a driver’s license or identification card.
