Oklahoma Consent or District Judge Option Form is a document used to establish paternity for a child born to unmarried parents in Oklahoma. The form allows the mother and legal father to agree to sign a Voluntary Acknowledgment of Paternity (VAP) without having to go through a court process. It also allows a district judge to sign the VAP if the mother and father cannot reach an agreement. There are two types of Oklahoma Consent or District Judge Option Form: a single-parent form and a two-parent form. The single-parent form requires the signature of the mother and the district judge; the two-parent form requires the signature of both parents and the district judge. Both forms must be completed, signed, and notarized before they are submitted to the court for filing. Once the forms are processed and filed, the paternity of the child is legally established.
