The Oklahoma Civil Cover Sheet (JS-44) is a form used to provide the court with information regarding a civil case. It is required to be completed and filed with the court when initiating a civil action in the state. This form is available on the Oklahoma State Courts Network (OCN) website and must be printed from the website to be accepted by the court. The Oklahoma Civil Cover Sheet (JS-44) consists of three parts. Part One requires basic information about the parties to the case, such as the names of the parties and the court in which the case is being filed. Part Two asks for information about the nature of the case, such as the type of action, claims, and relief sought. Part Three asks for information about the applicable law, such as the governing law and jurisdiction. The Oklahoma Civil Cover Sheet (JS-44) comes in two different versions, one for General Civil cases and one for Small Claims cases. For General Civil cases, the cover sheet must be filled out in its entirety and signed by the attorney or the plaintiff. For Small Claims cases, the cover sheet must be completed by the plaintiff only.
