The mother's document must include the mother's full name, date of birth, address and the name of the child, while the father's document must include the father's full name, date of birth, address and the name of the child. The joint parent document must include both parents' full name, date of birth, address and the name of the child. All documents must also include a statement of surrender and the signature of the parent or both parents, depending on the type of document. Once the document is signed, it is filed with the state court for review.
