Oklahoma Vocational Rehabilitation Services (VRS) Registry Form is a document used to establish eligibility for vocational rehabilitation services. It is an application used to collect information about the applicant's medical history, work history, income, education, and other relevant information. The form is used to determine if an individual qualifies for the services provided by the Oklahoma Vocational Rehabilitation Service. There are three types of Oklahoma Vocational Rehabilitation Services (VRS) Registry Form: the VRS Registry Initial Application; the VRS Registry Update Application; and the VRS Registry Verification Request. The VRS Registry Initial Application is used when an individual is applying for services for the first time. The VRS Registry Update Application is used when an individual is updating information previously provided on the initial application, such as medical history, work history, income, education, and other relevant information. The VRS Registry Verification Request is used when an individual requires verification of the information provided on the application.
