Oklahoma Prior Claims Request — Nonexempt Requestor is a form used by individuals to request a review of prior claims and/or claim history with the Oklahoma Department of Human Services (DHS). This form is used to identify and document prior claims, disputes, or other issues that could affect the current claim. It is used by individuals who are not exempt from the Oklahoma Prior Claims Law, which requires that individuals provide information about past claims and/or disputes with DHS prior to filing a new claim. There are two types of Oklahoma Prior Claims Request — Nonexempt Requestor: 1) Initial Request and 2) Follow-up Request. The Initial Request is used to submit the request for a prior claims review and the Follow-up Request is used to provide additional information or clarification on the initial request.
