The Oregon Declaration for Mental Health Treatment is a legal document that allows individuals to make decisions about their own mental health treatment. It allows individuals to give legal directions to mental health providers regarding treatment and care, including the right to refuse treatment. This document is also known as a mental health advance directive. The Oregon Declaration for Mental Health Treatment can be used to express treatment preferences in advance, in the event that the individual becomes unable to make decisions for themselves due to mental health issues. There are two types of Oregon Declaration for Mental Health Treatment: an Outpatient Directive and an Inpatient Directive. An Outpatient Directive is used to provide directions to mental health providers for outpatient treatment, while an Inpatient Directive is used to provide directions to mental health providers for inpatient treatment. Both types of directives must be completed and signed by the individual and a witness, and must be kept on file with the individual's mental health provider.
