Oregon Creditor Change of Address is a form required by Oregon law for creditors to update their contact information with the Oregon Department of Justice. This form is used to inform the state of any changes in the creditor’s address, phone number, or email address to ensure that the creditor receives any notices or documents from the Department. There are two types of Oregon Creditor Change of Address forms: (1) Initial Creditor Change of Address and (2) Subsequent Creditor Change of Address. The Initial Creditor Change of Address form is used to update the information for a creditor who has already registered with the Oregon Department of Justice. The Subsequent Creditor Change of Address form is used to update the information for a creditor who has registered more than once. Both forms require the creditor’s name, business address, phone number, and email address. The Initial Creditor Change of Address form also requires the creditor’s Oregon corporate registration number.
